Vos won the 133-kilometer stage from Cuenca by easily defeating Emma Norsgaard in a sprint finish in Guadalajara, giving her 248 victories overall.
The 35-year-old is currently 25 seconds ahead of Chloe Dygert, while Riejanne Markus, another member of Vos' Jumbo-Visma squad, is in third place, another second back.
With a sixth-place finish, Lizzie Deignan of Britain maintained her comeback from injury.
The 34-year-old, who delivered her second child in September and had just recently returned to competition, assisted her Trek-Segafredo team in stopping a number of assaults late in the stage while still having enough strength to place in the top ten.
After a three-rider breakaway was caught with 35 kilometers remaining, American Dygert and Dutch rider Annemiek van Vleuten attempted some late moves, but Vos won the stage for the second time in a row.
The fifth stage on Friday is 129 km from La Cabrera to Riaza's Mirador de Penas Llanas.
It features a summit finish and the category-one Puerto de Navafria climb, which is anticipated to upset the general classification standings prior to the weekend's mountain stages.
